2010 TEAM ENTRY

• Team Captains must read Registration Info before submitting entry.

• Complete one (1) Team Entry registration electronically. After typing in all team info, click 'submit' button.

• A printable version of your registration will appear. (A team code in the upper right corner of the form will appear for internal use). Click the 'Print' button.

• Print the form and mail with full team fee.

• (Individual team members register via the online roster (once accepted)).
